Patient Information and Forms
You are welcome to download, print and fill out your paperwork prior to your first appointment.
*Please complete all 5 forms.
Your First Visit
After your paperwork is complete, you can expect a thorough, interactive evaluation from your Physical Therapist. Your Physical Therapist will customize a treatment plan that will help you meet your functional goals. During this evaluation you will have the opportunity to discuss your condition, set goals for your therapy, and gain a good understanding of what to expect from attending physical therapy. You will also receive physical therapy treatment during this session, which may include modalities such as electrical stimulation, ultrasound, heat and/or ice, exercise, massage, joint mobilizations, and/or stretching. You should dress comfortably. Expect your first visit to last approximately 1 ½ hours.
Follow Up Visits
We will try to keep you with the same therapist every visit. If your schedule does not allow for this, your primary therapist will talk to your rendering therapist and exchange dialogue regarding your treatment plan. Expect your follow up visit to last approximately 45 minutes to 1 ½ hours.
On your first visit, our office manager will review your insurance benefits with you. Every plan is different, so we will let you know if you have a deductible, co-pay, or co-insurance due. It is your responsibility to let us know ASAP if you change plans during the course of your treatment.
If you do not have insurance, you have the option to pay out of pocket. Please contact our office for more information.
Most insurances do not require a prescription to come to physical therapy. Our physical therapists can diagnose and treat you. If you are coming to physical therapy with a prescription from a physician, we will communicate our findings with your physician and send him/her a copy of your initial evaluation. Please let our staff know if you have any follow up appointments scheduled with your physician so that we can send him/her an updated progress note.
Attendance and Compliance
It is important for you to realize that active participation in your health care is the key to a successful rehabilitation experience. One of the most important things you can do to speed up your recovery is attend your scheduled appointments. We understand that situations will arise that make it difficult to keep all of your appointments. If you are unable to make an appointment, we ask that you give us at least 12 hours notice of cancellation. If we are not given appropriate notice, you will be charged $65 for a missed appointment, once you have missed more than two appointments.
